Health Requirements for New Students

IMMUNIZATIONS
Every new student to the district must have a TB skin test (with results), done within the 12 months prior to enrollment. All immunizations must be taken to the Grant County Health Department, 105 S. Glenn, for analysis and record keeping purposes. The Grant County Health Department will give each parent a green sheet of paper with a stamp/initials showing evidence of their visit to the health department to present to the school district. New students will NOT be enrolled without this form showing that all immunizations and TB skin test are complete.

PHYSICALS
A physical examination done by a licensed physician, by a person acting under the direction of a licensed physician, or a nurse who has completed the Kansas Department of Health and Environment training and certification will be required for any student up to age nine enrolling in USD214 schools and who is new to Kansas schools. This needs to be completed no more than 6 months prior to enrollment and given to the school no later than 30 days after enrollment. The student may be excluded from attendance if noncompliant.

A student who plans on participating in school sports, grades 7-12, must have a sports physical completed after May 1st of the current school year. The physical needs to be on a KSHSAA (Kansas State High School Activities Association) Pre-Participation Evaluation Form.
